This form is long. It asks for everything a copyright notice must legally contain, and a number of things that help us verify it. Please set aside fifteen minutes.
Notices must be submitted in English. The statements you will be asked to swear to are legal text and are shown in English only, deliberately, because a translated version of them would not be the same statement.

The complaining party

Who you are, and how we can reach you. A notice that does not carry a name, a postal address and a telephone number is legally defective under 17 U.S.C. § 512(c)(3)(A)(iv), and we are not able to act on it. Everything in this section is treated as confidential and is never published on our website.

Your full legal name

As it would appear on a legal document. Not a username, a handle, or a company name.

In what capacity are you sending this notice?

I am the owner of the copyright (or of an exclusive right in it). I am authorized to act on behalf of the owner of the copyright.

Full legal name of the copyright owner you represent

Required if you are acting as an agent. The legal entity, not a brand or an app name.

The basis of your authority to act for that owner

For example: in-house counsel, a named anti-piracy vendor under written contract, or the owner’s exclusive licensee for this territory.

Your job title or position

For example: General Counsel, Head of Content Protection, Managing Director, Sole Proprietor.

Organization or company legal name optional

Leave blank only if you are an individual acting entirely on your own behalf.

Company registration number optional

Optional. The number your organization is registered under (for example a Companies House, EIN, SIRET, or HRB number).

Optional, but it is the fastest way for us to confirm you are who you say you are.

The copyrighted work you own

The original work — not what appears on our site, which is the next section. If you have trouble identifying the work precisely here, that is usually a sign the notice needs more preparation before it is sent.

Type of work

This form handles copyright only. Trademark and brand complaints are a different process — see the note at the end of this section.

Title of the copyrighted work

The original title of the work you own, exactly as it was published.

Original platform, studio or distributor optional

Where the work was first released. For example: ReelShort, DramaBox, Netflix, or your own studio’s name.

Year of first publication optional

Four digits. Approximate is acceptable if you are not certain.

Country of first publication optional

Optional. For example a U.S. Copyright Office number (PA/SR/TX). Leave blank if the work is unregistered — registration is not required for a valid notice.

A URL where the authorized original can be viewed

The official store page, streaming page, catalogue entry or press page for the work. If the work has never been published anywhere online, write "unpublished" and explain below.

Enough detail for someone who has never seen it to identify it: what it is, how long, who made it, when, and what makes the copy on our site a copy of it.
